Family Luggage Tag Information Form

Please fill out the information in the order form and submit through our secure server.  We need your email address in case there is a need to contact you. 

Picture(s) will be on the back of the card.  If you are ordering more than one card, you may have a different picture on the other tags.  So if you are ordering five (5) tags, each tag can have a different picture.  Note: You are limited to 2-3mb per file (picture) and a total of 20mb per form. 2mb per photo or less works best.  It may take a while to load five images, so be patient.

You cannot change text for the front of the tag.

We reserve the right to refuse any photo not suitable for publication or viewing.
